The mission of MedLink Georgia is to provide quality, comprehensive, individualized diabetes self-management education to the patients we serve. We believe that education is the key to empowering the person with diabetes to better manage his or her disease and avoid the complications of diabetes and achieve an optimum health status. MedLink Georgia places a special emphasis on offering access to diabetes education to the underserved and high-risk populations in our community who are impacted by diabetes and aligns with our vision of “healthy individuals, healthy families, and healthy communities”.
Our nationally accredited program provides education, training, and support to patients diagnosed with Type I and Type 2 Diabetes, Maturity Onset Diabetes in Youth (MODY), Latent Autoimmune Diabetes in Adults (LADA), Gestational Diabetes, Impaired Glucose (Pre-Diabetes) and Metabolic Syndrome.
Those referred to our program will partner with the Registered Dietitian and/or the Diabetes Care and Education Specialist to develop an individualized plan that encourages and supports the patient in adopting healthy lifestyle choices. Our educators will work with the patient, the patient’s family, and the patient’s physician to prevent long-term complications, improve diabetes management and blood glucose levels, and improve the quality of life. To participate in this amazing program, a physician’s referral is needed.
